In order to register your student with K.E.Y.S. of AZ, you, the Parent and/or Legal Guardian, must agree to the following terms and conditions.
By joining K.E.Y.S. of AZ, I understand that my student and I will be asked to sign and adhere to all of the policies outlined in the K.E.Y.S Student Covenant and the K.E.Y.S. Parent Covenant.
I, the Parent and/or Legal Guardian of my registered student hereby release K.E.Y.S. of Arizona, and Christ's Community Church, located at 4530 W. Thunderbird Rd., Glendale, Arizona 85306, their leaders, their chapter, officers, directors, members, employees, private contractors, and all volunteers from any and all liability for damage or injury to my child or myself, or to any person or property during the time of my child's attendance at K.E.Y.S. of AZ sponsored events from any claims, including the cost of expenses arising in connection with injuries or damages sustained by my child, me, or any member of my family.
Furthermore, I assume full responsibility for my child's conduct and for any damage or injury caused by my child's actions.
By registering and enrolling my child for any class or activity with K.E.Y.S. of AZ, I understand that I am liable for the entire semester of tuition per class and the non-refundable registration fee, regardless of my child's attendance. I also understand that tuition is due on the 1st of each month from September through March. In compliance with the K.E.Y.S. Terms and Conditions, I agree to pay an $11.00 per class late fee if my tuition is received on or after the 5th day of each month. If tuition is more than one month in arrears, I under that my student will not be permitted to attend class until payment is received unless prior arrangements have been made.
